Advance Care Planning Australia is an Australian Government initiative administered by Brisbane South Palliative Care Collaborative at Metro South Health. We aim to promote and improve advance care planning for all Australians.
What is advance care planning?
Advance care planning involves planning for your future health care. It allows you to decide what health care you would or wouldn't want if you become unable to make or communicate your treatment preferences in the future.
Our vision
Our vision is for people to:
- have a say in their future health care
- record and share their wishes and preferences
- be heard by their care providers
- have their wishes and preferences respected.
Our objectives
We aim to:
- help everyone understand advance care planning and improve participation
- educate the health and aged care workforce about advance care planning
- improve the systems that support advance care planning
- build and share knowledge through research and evidence.
