This submission by PCA to the Primary Health Care Advisory Group provides input into the review of primary health care. This submission is the product of wide spread consultation from PCA member organisations and stakeholders, and discusses Better Outcomes for People with Chronic and Complex Health Conditions through Primary Health Care. PCA provides seven recommendations, including supporting case conferencing, supporting telehealth provision, and the development of advance care plans for people with chronic progressive disease.
