Palliative care and chronic heart failure 

A joint position statement from PCA and the Heart Foundation.

Palliative care should be available to all people diagnosed with life-limiting conditions, including chronic heart conditions.

PCA and the Heart Foundation call for improved awareness among health professionals and the community, of the benefits of palliative care for a person with chronic heart disease.

Early identification and timely referral to palliative care as well as a collaborative approach are important to decision making and person-centred palliative care for those with heart conditions.
