Victoria

In Victoria, the three components of an Advance Care Plan are:

1. Enduring Power of Attorney (Medical Treatment)

A legal document for appointing a substitute decision maker with the power to make decisions about medical treatment on your behalf.

2. Advance Care Plan/Directive

A document expressing  your values and preferences to inform future medical treatment and decision making if you become unable to participate.

3. Refusal of Treatment Certificate

Documentation of legal refusal of medical treatment generally or of a specific kind, in relation to a current condition.
